Care Navigation

Our reception staff have all been trained in Care Navigation.  When you telephone the practice for an appointment, they will ask you some relevant questions in order to be able to direct you to the most appropriate health care professional within the practice.  For example, a Nurse Practitioner, Minor Illness Nurse or Pharmacist or in some cases outside of the practice to a pharmacist, direct physiotherapy, optician, dentists, counselling and some social care.

They do not offer clinical advice or triage but this new way of working is about offering choice to see other specialists within the practice tea

Care Navigation at Bryngwyn Surgery

All our reception staff are trained in Care Navigation to help ensure you receive the most appropriate care as quickly and efficiently as possible.

When you contact the surgery, our Care Navigators will ask a few brief and relevant questions about your needs. This is not a clinical assessment or triage — it’s simply to ensure you are directed to the most suitable member of our healthcare team.

Depending on your symptoms, you may be offered an appointment with:

  • A GP

  • Our Advanced Paramedic Practitioner

  • Our Clinical Pharmacist

  • One of our Specialist Nurses

  • Or a healthcare professional outside of the practice (e.g. community pharmacy, direct access physiotherapy, optician, dentist, counsellor, or social care service)

This approach means you won’t always need to see a GP first — helping us reduce waiting times and improve access while ensuring you get the right care, from the right person, at the right time.

You always have the right to request a GP appointment, but we may suggest an alternative if it’s more appropriate for your specific concern.
