Under the Freedom of Information Act 2000, GP practices are required to make certain information publicly available.
Bryngwyn Surgery has adopted the Information Commissioner’s Office (ICO) Model Publication Scheme, which is a guide to the information routinely published by public authorities.
This scheme commits the practice to making information available to the public as part of our normal business activities.
The Model Publication Scheme groups information into the following categories:
Information about the practice, including:
practice contact details
opening hours
details of doctors and staff
organisational structure.
Financial information relating to the practice, including:
funding received from NHS Wales
financial policies and procedures.
Information about practice performance and service development, including:
quality improvement work
practice plans and priorities.
Information about how decisions are made within the practice, including:
practice policies
procedures and protocols.
Policies that guide the way the practice operates, including:
confidentiality and data protection policies
complaints procedures
safeguarding policies.
Information that the practice maintains in registers, where appropriate and lawful.
Information about the services provided by the practice, including:
clinics and services available
patient information and leaflets.
If you cannot find the information you require on our website, you can submit a Freedom of Information request.
Requests must:
be made in writing (email is acceptable)
include your name
include an address for correspondence
clearly describe the information you are requesting.

The Freedom of Information Act does not cover personal data, including patient records or staff personal information. These are protected under Data Protection legislation.
Requests for access to your own medical records should be made as a Subject Access Request.
