We are committed to continuity of care, offering you access to your preferred clinician wherever we can. We know this improves patient outcomes and builds trust, and we’re working towards making it the norm, not the exception.

  • Treat all staff with respect – we’re here to help, and everyone deserves a safe, courteous working environment.
  • Keep appointments or cancel in good time so we can offer the slot to someone else.
  • Let us know of any changes to your address or phone number so we can stay in touch.
  • Follow agreed treatment plans and seek clarification if you’re unsure about any medical advice.
  • Respond promptly to letters or requests for contact.
  • Understand that delays happen if another patient has a medical emergency – we will always give you the time you need.
  • Take part in health promotion and screening programmes when offered.
